To help your appointment start as quickly as possible, we’ve made our patient forms available as a download below. Simply click on the link and fill out the forms ahead of your visit. That way, when you come to see us, you can just give us the completed forms, and we can get you in to see Dr. Anderson right away. If you have any problems accessing these forms, please let us know right away!
Anderson Family Dental is happy to work with your dental insurance to maximize your benefits. We are in-network with Delta Dental Premier, United Concordia, Cigna, and BlueCross BlueShield, and we’ll also accept any plan that allows you to choose your dental care provider. Our team will be able to answer any insurance questions and let you know how we can use your particular plan to help you save at your next visit—just give us a call today.
Outside of dental insurance, we accept cash, checks, Discover, Visa, MasterCard, and CareCredit. CareCredit makes it possible for you to finance your dental care, providing no-interest and long-term payment plans for those who qualify. It’s an excellent way to make your care more affordable if you currently don’t have insurance or require extensive treatment. You can go over their different plans and even sign up for one by clicking on the link below. If you would like any help with the application process, please give us a call.
